We recommend downloading directly from the DOJ to ensure you have the current version. Forms are revised periodically, and an out-of-date form can be rejected.
Federal forms

| FD-258 | Standard FBI fingerprint card. We print these for you. You do not need to bring one. |

California Live Scan: applicantstatus.doj.ca.gov. You will need the ATI number from your appointment receipt.

Which form is which?
BCIA 8016 covers almost everyone: jobs, licenses, permits.
BCIA 8016A if you work for a public school district or JPA.
BCIA 8016VECHS if you are a volunteer with a qualified entity.
BCIA 8016RR if nobody asked. You just want to see your own California record. State fee $25.

Remember
A blank form is not enough on its own. The requesting agency has to fill in the ORI code and applicant type. Without those we have nowhere to submit your prints.
