What to Bring to Your Appointment

Two things prevent almost every failed appointment. Please check both before we arrive.

Your Request for Live Scan Service form

The agency requesting your background check gives you this form. We cannot create one for you and we cannot fingerprint you without it.

Your form must include:

  • The ORI code of the requesting agency
  • The Authorized Applicant Type
  • The type of license, certification, or permit, or your working title
  • The requesting agency name and mailing address

Bring three copies if your agency requires it. Many do. Check your instructions.

Forms come in a few versions: BCIA 8016 (standard), BCIA 8016A (public schools), BCIA 8016VECHS (CalVECHS volunteers), and BCIA 8016RR (requesting your own record). Download forms

Text or email us a photo before your appointment and we will check it for free. This takes two minutes and saves you a wasted appointment.

Valid, unexpired government-issued photo ID

Accepted

  • California driver license
  • California DMV identification card
  • Out-of-state driver license
  • U.S. passport
  • U.S. military ID (active duty, retiree, or reservist)
  • Permanent Resident Card (I-551)
  • Employment Authorization Card (I-766)
  • Foreign passport with appropriate immigration documentation

Not accepted

  • Expired ID or driver license, even by one day
  • Temporary or paper interim licenses
  • ID with a hole punch
  • Photocopies or photos of ID on your phone
  • School or work badges

This is a State requirement, not our policy. We are not permitted to fingerprint you without acceptable identification.

Payment

Credit card, debit card, cash, or company check. Businesses with an approved account can be invoiced.

For an FBI Identity History Summary

  • Valid, unexpired government-issued photo ID
  • A working email address
  • A phone that can receive a text or call. Your results are protected by two-factor authentication and you cannot retrieve them without it

Your email address must be exact

Results are sent to that address and only that address. A typo means the submission has to be redone at additional cost, so we will read it back to you before we transmit.

You will also complete a short federal form (the Departmental Order form) at your appointment. We provide it.

Before we arrive

  • Have your form and ID out and ready
  • Wash and dry your hands. Clean, moisturized hands scan better. If your hands are very dry, applying lotion the night before genuinely helps.
  • Remove bandages from fingertips if possible

Cuts, scars, or worn fingerprints?

It happens, and it is not a problem. We work with what you have and use the appropriate capture technique. If a submission is rejected for print quality and we processed the original, we come back and re-roll at no charge.

After your appointment

You will receive a copy of your form with your ATI number. Keep it. That number is how you track your submission.

Results go directly to the agency that requested them. We never see or receive your results, and we cannot look them up for you.
